Soccer Registration Fees and Process

REGISTRATION DOCUMENTS

The 5 Documents attached below must be downloaded, printed out, filled out, signed, and then uploaded (scan or take a picture) back to your computer BEFORE starting the Registration Process.

All 5 Forms will be required to upload DURING the Registration Process in order to complete registration.

Both Physical Forms must be printed and signed by the physician, then uploaded into Player Registration.

Sports physicals are only good for one year.

Sports Physicals are $25 at HealthFirst Rapid Care.
